    Most common source for vulnarities are website scripts & contact forms.
    In cases when your IP gets blacklisted can cause the emails spams. You can check whether you IP is blaclisted from here

    i remove it ok, but mailserver continue send mail outside so blacklist again

    i thing the firt : stop mail send outside , but when restart service postfix , it send again and my ip blacklist again.

    I would not do that. It's kinda risky. 454 is the least I would use. 554 is safest, one would assume the lists you use are reliable enough for that. And if they're not, you'll hear soon enough, because senders get their mails bounced.
