Recent install - I'm finding that some mail comes through with out X-Spam-Flags on it.

I received the same email to 2 separate email addresses - 1 had X-Spam headers & 1 did not

Copy of the full header & Below is a link to the debug log from Amavis for the 2 emails with/with out X-Spam-Flag

    No spam flags

    Spam flags

The spam flag is set if the email is recognized as spam. So if no spam flag is set, spamassassin did not recognize this email as spam.

Ok, but how did spamassassin recognize the email in 1 instance as spam & in the 2nd instance as not spam?

It is exactly the same email sent to 2 different email addresses in the same domain.
