Clone existing server to new server

What are the correct steps to backup the current installation to a new server?
Untill today I can't find 1 single post where this is covered and explained. I found some different posts but very unclear what to backup and the steps to take to end up with a working new server.

I would like to suggest to make a sticky post about backup/restore/clone a server.

If the new server will have the same IP, you can create an image of your server (e.g. with CloneZile or Ghost4Linux) and restore it on the new server.

The old server has an older version of Ubuntu installed.
I would like to upgrade to version 12.
