I continue to have a problem with ONE user on ONE domain on my server. I have deleted his account and rebuilt it and continue to have this problem ONLY with this user. mate i had the exact same problem, multiple domains, multiple users, what i narrowed it down to was that even though the users have an unlimited quota... the user that wasnt receiveing the emails had a large amount of emails in their inbox, i created a new folder (Just through email client) moved all the inbox mess into the new folder and i was able to send mail from an external domain, very weird, let me know if your problem is the same as mine.........
