PHPMyAdmin - Block from WAN, only to be accessed by internal 192.

Hi,
Recently i had a successful breach to phpmyadmin from outside my LAN hosted on one of my domains powered by ISPConfig.
All domains i host all have access to the login page of phpmyadmin from anywhere outside of the LAN.
How do i stop this?
Can i specify specific MAC or IP Addresses to only be able to access *.domain/phpmyadmin, if so, what is the rule and which files need to be modified?

Redirect any request that requests: Http://anydomain.com/phpmyadmin to Http://anydomain.com/404-Error.html
