Survey - free webapp

Hi folks,

I am looking for some survey webapplication, in best cases non java stuff to aviod running a tomcat or similar for this purpose.
It should be pretty flexible towards designing the questions / dependant questions and a fitting engine to render the result. Also it should support anonymous and non-anonymous surveys.
