KMail behaves like that in Kde 3.3 and below. I've been told that in Kde 3.4 KMail is able to show a paper clip for emails with attachments....
It's used by e2fsck (similar to scandisk in MS Windows) for orphaned data blocks (if the filesystem is corrupt). If e2fsck finds corrupt files...
